Leave Of Absence Request

Memberships may be placed on hold for full months. Minimum of one month and a maximum of 3 months per calendar year. Monthly dues of $25 per month will be charged to maintain your account.

Please note: any future modifications to this membership will result in an assessment of dues at the rate that is in effect at the time of change.
